Where your answers go

The eligibility survey opens on Mass General Brigham's REDCap system at redcap.partners.org. This website does not receive or store your survey answers. The form asks for your name, email, phone number, age, and smoking history. Its introduction says ALCSI's team uses contact details to assist with screening information and will contact you only if you choose follow-up. Your decision to open the form is not a medical or research consent.

Before submitting

Read the REDCap form's own introduction and any notice it presents before you enter information. The public form does not state a retention period or identify a specific data steward. To ask who can access responses or how long records are kept, contact ALCSI at info@alcsi.org or call 1-844-YES-LUNG before submitting.

Clinical care

The survey can help start a conversation about screening. It cannot tell you that you qualify, guarantee insurance coverage, or replace a clinician's recommendation. A clinician can review your history and help you weigh the benefits and harms of low-dose CT screening.
